Terms and Conditions

I, being the parent or guardian of ("Camper"), a minor, hereby agree to the following:
a. Camper is in good health, with no present medical or psychiatric conditions that would prevent attendance and full participation at an ALLBALL! Inc. Camp.
b. Camper has health insurance.
c. I hereby release and hold ALL BALL!, its shareholders, directors, officers, and employees harmless from any and all injuries, accidents, damages or losses that Camper may incur whether from natural or manmade causes, foreseeable or unforeseeable, expected or unexpected.
d. I am aware that the engaging in recreational and sports activities at the camp may be a hazardous activity. Camper is voluntarily participating in this activity with knowledge of the danger involved and I hereby agree to accept and assume any and all risks of injury associated therewith including, but not limited to, property damage, personal injury, bodily injury, or death. e. As consideration for being permitted to attend and participate in an ALL BALL! camp, I hereby agree that I, on behalf of myself and Camper, will not make a claim against, sue or attach the property of or make any other demand on ALL BALL!, its shareholders, directors, officers, and employees or any of their affiliated organizations for injury or damage resulting from negligence or gross negligence or other acts, howsoever caused, by any employee, agent, or contractor of ALL BALL! or any of their affiliated organizations as a result of Camper's attendance and participation at an ALL BALL! camp. I hereby release ALL BALL!, its shareholders, directors, officers, and employees and any of their affiliated organizations from all actions, claims, or demands that I now have or may hereafter have for injury or damage resulting from attendance and participation at an ALL BALL! camp. f. I agree that ALL BALL! may render immediate emergency medical treatment and/or transport Camper to a medical care facility. g. ALL BALL! have my permission for photographs and/or videos to be taken of Camper and ALL BALL! has the right to utilize such materials in brochures and other promotional materials. h. I agree that Camper must abide by camp policies and the instructions of the camp staff. I agree that should Camper be dismissed from camp, no part of the tuition shall be refunded. i. I agree that no part of my tuition shall be refunded for late arrivals, early departures, vacations, illness or injury. CORONAVIRUS/COVID-19 ASSUMPTION OF RISK AND RELEASE OF LIABILITY The novel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization. COVID- 19 is extremely contagious and is believed to spread mainly from person-to-person contact. Allball Basketball Camps has put in place preventative measures to reduce the spread of COVID-19 at its summer youth programs. However, Allball Basketball Camps cannot guarantee that you or any minors in your care will not become infected with COVID-19. Further, participation and attendance at the Allball Basketball Camps could increase your, or any minors in your care, risk of contracting COVID-19. READ CAREFULLY BEFORE SIGNING (CLICKING "I AGREE") By signing this agreement, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk for myself and/or for any minors in my care that may be exposed to, or infected by, COVID-19 by participation in Allballl Basketball Camps and that such exposure or infection may result in personal injury, illness, permanent disability, and death; I understand that the risk of becoming exposed to, or infected by, COVID-19 at Allball Basketball Camps may result from the actions, omissions, or negligence of myself and others, including, but not limited to, Allball Basketball Camps, its employees, volunteers, agents, representatives program participants and their families; I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to myself and any minors in my care: including, but not limited to, personal injury, disability, death, illness, damage, loss, claim, liability, or expense of any kind, that I or minors in my care may experience or incur due to COVID-19 in connection with my participation in Allball Basketball Camps; On behalf of myself and minors in my care, I hereby release, covenant not to sue, discharge, and hold harmless Allball Basketball Camps, its employees, facilities, volunteers, agents, and representatives, of and from the Claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto. I understand and agree that this release includes any Claims based on the actions, omissions, or negligence of Allball Basketball Camps, its employees, volunteers, agents, and representatives, whether a COVID-19 infection occurs before, during, or after participation in Allball Basketball Camp. By clicking submit, I agree that if I, or a minor in my care, is exposed or infected by COVID-19 during participation in Allball Basketball Camps, then I, or any minor in my care, may be found by a court of law to have waived my right or the rights of any minor in my care to maintain a lawsuit against the parties being released on the basis of any claim for negligence.
